Skeletal System: Bone Growth and Remodeling

The skeletal system undergoes significant changes throughout an individual's life, particularly during bone growth and remodeling. Understanding these processes is essential for grasping how bones develop, maintain their structure, and adapt to various stresses.

Bone Development

Bone development, or ossification, is the process through which bone tissue is formed, beginning as early as the second month of fetal development and continuing with postnatal growth until early adulthood. Bone remodeling and repair are lifelong processes, allowing the skeleton to adapt to new stresses and repair itself after injuries.

Formation of the Bony Skeleton

During early development, the fetal skeleton, consisting of fibrous membranes and hyaline cartilage, is gradually replaced by bone tissue. There are two primary types of ossification:

Endochondral Ossification: Involves the formation of bone by replacing hyaline cartilage, primarily in bones below the base of the skull (excluding the clavicles). It begins late in the second month of development, utilizing pre-existing cartilage models.
Intramembranous Ossification: Begins within fibrous connective tissue membranes formed by mesenchymal cells, responsible for forming flat bones of the skull and the clavicle.

Steps of Endochondral Ossification

A bone collar forms around the diaphysis of the cartilage model.
The central cartilage in the diaphysis calcifies and develops cavities.
The periosteal bud invades these cavities, leading to the formation of spongy bone.
The diaphysis elongates, and the medullary cavity forms. Secondary ossification centers appear in the epiphyses.
The epiphyses ossify, with hyaline cartilage remaining only in the epiphyseal plates and articular cartilages.

Steps of Intramembranous Ossification

Ossification centers form when mesenchymal cells cluster and differentiate into osteoblasts.
Osteoid is secreted and subsequently calcified.
Woven bone forms as osteoid is laid down around blood vessels, resulting in trabeculae.
Lamellar bone replaces woven bone, and red marrow appears.

Postnatal Bone Growth

After birth, long bones grow in length through interstitial (longitudinal) growth at the epiphyseal plate and increase in thickness through appositional growth. Most bones cease growing during adolescence, although some facial bones may continue to grow slowly.

Growth in Length of Long Bones

The growth in length of long bones depends on the presence of epiphyseal cartilage in the epiphyseal plate, which maintains a constant thickness. The epiphyseal plate consists of five distinct zones:

Resting (quiescent) zone: Relatively inactive cartilage on the epiphyseal side.
Proliferation (growth) zone: Contains rapidly dividing cartilage cells, pushing the epiphysis away from the diaphysis.
Hypertrophic zone: Contains older chondrocytes closer to the diaphysis, with enlarged lacunae forming interconnecting spaces.
Calcification zone: Surrounding cartilage matrix calcifies, leading to chondrocyte death.
Ossification (osteogenic) zone: Chondrocyte deterioration leaves calcified cartilage spicules, which are eroded by osteoclasts and replaced with spongy bone by osteoblasts.

As individuals approach the end of adolescence, chondroblasts divide less frequently, leading to the thinning of the epiphyseal plate, which is eventually replaced by bone, marking the cessation of bone lengthening around 18 years in females and 21 years in males.

Hormonal Regulation of Bone Growth

Bone growth is regulated by hormones, primarily growth hormone, which stimulates activity in the epiphyseal plate during infancy and childhood. Thyroid hormone modulates growth hormone activity, while testosterone and estrogens promote growth spurts during puberty and contribute to the closure of the epiphyseal plate.

Growth in Width (Thickness)

As bones lengthen, they also widen through appositional growth, which can occur throughout life. Osteoblasts beneath the periosteum secrete bone matrix on the external surface, while osteoclasts remove bone from the endosteal surface, typically resulting in thicker, stronger bones.

Bone Remodeling

Bone remodeling is a dynamic process where approximately 5-7% of bone mass is recycled each week. Spongy bone is replaced every 3-4 years, while compact bone is replaced approximately every 10 years. This process consists of both bone deposition and resorption, coordinated by remodeling units of adjacent osteoblasts and osteoclasts.

Bone Resorption

Bone resorption is primarily the function of osteoclasts, which secrete enzymes and protons that digest the bone matrix. The acidity generated converts calcium salts into soluble forms, and osteoclasts phagocytize demineralized matrix and dead osteocytes, releasing products into the bloodstream. Activation of osteoclasts is influenced by parathyroid hormone (PTH).

Bone Deposit

Bone deposition is the function of osteoblasts, marked by the osteoid seam and calcification front. Triggers for bone deposition may include mechanical signals and increased concentrations of calcium and phosphate ions necessary for hydroxyapatite formation.

Control of Remodeling

The remodeling of bone is controlled by hormonal signals, particularly PTH, which is produced in response to low blood calcium levels, ensuring efficient bone growth, maintenance, and repair throughout life.

Response to Mechanical Stress

Bone structure and density are influenced by mechanical stress. According to Wolff’s law, bones adapt to the stresses they encounter, growing or remodeling in response to demands. This stress causes bones to bend, compressing one side while stretching the other, leading to thickening where bending stresses are greatest.

Handedness, with thicker bones in the dominant upper limb.
Curved bones, thickest at points likely to experience buckling.
Formation of trabeculae along lines of stress.
Large bony projections at sites of heavy muscle attachment.
Featureless bones in bedridden individuals or fetuses due to lack of mechanical stress.
Development of sesamoid bones within tendons.

Osteoporosis

Osteoporosis is characterized by bone resorption outpacing deposition, leading to decreased bone mass and increased fracture risk, particularly in the spine and femur.

Risk Factors for Osteoporosis

The risk is higher among aged, postmenopausal women, with approximately 30% affected by age 60-70 and around 70% by age 80. Other risk factors include:

Insufficient physical activity.
Low calcium and protein diet.
Smoking.
Genetic predisposition.
Hormonal conditions.
Excessive alcohol consumption.

Treatment and Prevention of Osteoporosis

Treatments include calcium and vitamin D supplementation, weight-bearing exercises, and hormone replacement therapy. Preventative measures focus on adequate calcium intake, reducing carbonated beverages and alcohol, and maintaining weight-bearing exercises.

Age-Related Changes in Bone

Bone density and structure change throughout life. In children and adolescents, bone formation exceeds resorption, while in young adulthood, they are balanced. After the fourth decade, resorption exceeds formation, leading to gradual bone density decline, with females generally experiencing more pronounced loss than males.
