Balanced polymorphism is a concept in population genetics that explains how certain deleterious genes can remain prevalent within a population despite their negative impact on survival and reproduction. At first glance, it appears that natural selection would continuously work to diminish the frequencies of detrimental alleles to low levels; however, the reality is more complex. Some harmful genes can exist at surprisingly high frequencies across various populations, one prime example being the gene responsible for sickle-cell anemia.

SICKLE-CELL ANEMIA

Sickle-cell anemia is a condition that manifests when an individual inherits two alleles for sickle-cell hemoglobin (Hb S), resulting in the production of abnormally shaped red blood cells. These red blood cells assume a crescent, or sickle, shape, which adversely affects their functionality, especially in navigating small capillaries. This leads to blockages that deprive tissues of oxygen, causing painful crises and significantly reducing life expectancy—many individuals with this condition do not survive past the age of 50.

The disease predominantly affects individuals from certain ethnic backgrounds, particularly people of African, Mediterranean, and Southeast Asian descent. In the United States, the incidence of sickle-cell anemia varies; for example, it occurs in about 2 per 1,000 live births among African Americans, while the rate in Hispanic Americans is about 1 in 35,000. Approximately 70,000 individuals in the U.S. are affected by sickle-cell disease, and around 2 million African Americans carry the sickle-cell trait, inheriting one normal allele and one sickle allele.

Individuals with the sickle-cell trait are generally healthy, but under certain stress conditions, their red blood cells can sickle as well, indicating that the gene exhibits a codominant relationship—both alleles are expressed in individuals who are heterozygous (Aa). The presence of sickle hemoglobin, which differs from normal hemoglobin due to a single point mutation where glutamic acid is replaced by valine, can be linked to a genetic advantage.

THEORY OF BALANCED POLYMORPHISM

The phenomenon of balanced polymorphism arises when a heterozygote possesses a selective advantage over both homozygotes—those with two normal alleles (AA) and those with two sickle alleles (aa). This advantage can stabilize the frequencies of both alleles in the population, maintaining the sickle-cell allele even when homozygous conditions result in severe health complications.

Research indicates that heterozygous carriers of sickle-cell traits exhibit increased resistance to malaria, particularly in regions where malaria is endemic. This resistance provides a survival advantage, allowing carriers to thrive, which sustains the sickle-cell allele in populations living in malarial zones. This selective advantage is particularly pronounced in agricultural communities, which are often densely populated with the malaria-causing parasite, Plasmodium falciparum.

Consequently, balanced polymorphism can maintain deleterious alleles due to the advantageous traits granted by heterozygosity. In various African populations, significant proportions of individuals carry the sickle-cell trait, leading to an equilibrium in allele frequencies that balances the disadvantages posed by homozygous genotypes.

SELECTION AGAINST THE HETEROZYGOTE

In another genetic condition, selection against heterozygotes can occur, as demonstrated by Rh disease, or erythroblastosis fetalis. This condition arises when an Rh-negative mother has an Rh-positive child, leading to potential immune reactions that can threaten the fetus's survival. In these cases, the mother produces antibodies against the Rh antigens present in the fetal blood cells, and subsequent pregnancies with Rh-positive offspring can result in severe complications for the baby.

Despite the risks, studies suggest that the prevalence of the Rh-negative allele (r) remains stable due to compensatory reproductive behaviors. Individuals who have lost children to Rh incompatibility often have larger families to counterbalance their losses, ultimately sustaining the frequency of the recessive allele. This stability of the r allele reflects the complex dynamics between selection pressures and reproductive strategies in human populations.

Among Caucasians in the U.S., about 16% are Rh-negative, and higher frequencies are reported in certain European populations, showing the diverse genetic landscape influenced by both natural selection and cultural practices.
