Understanding Cancer

Cancer, also referred to as a tumor or neoplasm, represents an abnormal growth of tissue that arises from uncontrolled and disorganized cell proliferation. It is initiated within a single cell by the accumulation of mutations in key genes responsible for proliferation, survival, and invasion. It's crucial to note that not all tumors are cancerous; benign tumors grow slowly and remain localized, while malignant tumors invade surrounding tissues and can metastasize.

Benign vs. Malignant Tumors

Benign Tumors

Non-cancerous cells.
Slow growth rate and encapsulated, remaining localized.
Do not invade surrounding tissues.
Retain normal cell specialization with typical cell shapes.
Rarely fatal and have a good prognosis.

Malignant Tumors

Cancerous and invasive cells.
Rapid growth rate and capable of invading other tissues.
Have potential to metastasize to distant organs.
Poorly differentiated and specialized, often displaying larger and atypical cell shapes.
Considered life-threatening, generally with a poor prognosis.

Classification of Tumors

Tumors are broadly classified based on their tissue of origin, with significant suffixes denoting specific types:

Carcinomas: Cover external and internal organs or glands.
Sarcomas: Involve supporting tissues like bone, fat, and muscle.
Lymphomas: Originate in lymph nodes and tissues of the immune system.
Leukemias: Arise from immature blood cells in the bone marrow.

Common Cancer Types

Common prefixes are used to designate types of cancer based on the tissue affected:

adenosarcoma: glandular tissue
chondrosarcoma: cartilage
hemangiosarcoma: blood vessels
melanoma: pigment cells.

Cancer Statistics

In the UK, the incidence of cancer was recorded at 385,000 new cases in 2019, leading to approximately 167,100 deaths. It is noted that around 38% of these cancer cases are preventable, highlighting the importance of awareness and early intervention.

Hallmarks of Cancer

The hallmarks of cancer, as identified by Hanahan and Weinberg, include the following:

1. Sustaining Proliferative Signaling

Cancer cells exhibit autonomous growth by becoming hyperresponsive to growth factors, amplifying signals that promote cell division.

2. Evading Growth Suppressors

Cancer cells can ignore signals that would normally inhibit their growth, often through genetic alterations that silence tumor suppressor genes.

3. Resisting Cell Death

Cancer cells develop mechanisms to evade apoptosis, often through mutations in pro-apoptotic or anti-apoptotic regulators.

4. Enabling Replicative Immortality

These cells maintain lengthy telomeres through the action of telomerase, allowing them to bypass normal limits to division.

5. Inducing Angiogenesis

To sustain their growth, tumors stimulate the formation of new blood vessels, ensuring a constant supply of oxygen and nutrients.

6. Invasion and Metastasis

Cancer cells can detach from their local environment and invade surrounding tissues or spread to distant sites, leading to secondary tumors.

Deregulating Cellular Energetics: Cancer cells alter their energy production to support rapid growth.
Avoiding Immune Destruction: Tumor cells can evade detection and destruction by immune cells.
Genome Instability: Alterations in DNA repair processes contribute to genetic variations and malignancy.
Promoting Inflammation: The inflammatory response can supply a favorable environment for tumor growth.
